Care Management on the Support at Home Statement

Care Management will display on the statement in the detailed expense information section for the budget it was linked with.

If there are multiple appointments for each section, lines will be consolidated as follows:

  • The ongoing service care management lines will be consolidated based on rate, with units added up.

  • The restorative care management and end of life care management lines will be consolidated based on service, rate and unit cost, with units, total cost and subsidy added up.
